The Model Release Form contract template includes FOUR OPTIONS to use depending on whether you are depicting your model as an imaginary person or whether the model is to be depicted as a real person with their name/image/likeness involved, and whether your models are minors or adults. Each option covers all of these legal bases for you:

However, The Legal Paige® disclaims having your specific contract reviewed by an attorney in your state to ensure full compliance with local laws.

Please note that all of our contracts, unless otherwise noted, are U.S. based and not suitable for use outside the U.S. (If you reside in the U.S., you are considered U.S.-based). If you are located outside of the U.S. or Canada, we advise consulting a licensed lawyer/esquire/barrister in your country to review the contracts, as the language will need to be modified to comply with your country’s contract laws.

How will I receive the contract after purchase?

You’ll receive a .zip folder with a 5-download limit.
